New Patients

New patient medical information forms are completed with our unique NextPEN that interfaces with our EHR (electronic health record) at the time you check in for your first appointment. It is imperative that you arrive at least 30 minutes prior to your scheduled appointment time. Failure to do so may likely result in you having to reschedule your appointment. We ask that new patients complete the following forms and bring them with you to your first appointment:
Other items to bring to your new patient appointment:
- Advance Directive forms (Healthcare Proxy/Durable Power of Attorney) to your appointment for our records.
- Your current insurance card for accurate billing purposes.
- If your insurance requires a referral it is your responsibility to make sure that this is in place prior to your appointment. Failure to do so may result in us having to reschedule your appointment.
- Be prepared to pay your co-pay, if applicable, at your appointment. We accept cash, checks, and all major credit cards.

If you fail to show, or fail to provide 24 hour advanced notice that you need to reschedule, we reserve the right to charge your account a $50 No Show Fee. In addition, we reserve the right to refuse to reschedule a new appointment for you in the future. As a courtesy to the physician who referred you to our practice, we will notify them of your failure to show.
